NTSB release preliminary report on Katz crash; Philadelphia Fire Commissioner Lloyd Ayers retires

The National Transportation Safety Board has released a preliminary report on the airplane crash that killed Lewis Katz and 6 others outside Boston on May 31st. The report indicated that the flight crew did not perform a flight control check prior to taking off. It also indicated that there may have been a problem with the mechanical gust lock system. The system protects airplane control surfaces from wind gusts when the airplane is parked, but would prevent an airplane from taking off if not fully disengaged. If the crew performed their preflight check they could have discovered a problem with the gust lock. Flight recorders show that the airplane reached takeoff speed but never lifted off. It also showed that brakes and reverse thrusters were engaged, however it was too late. A full report could take months.

Philadelphia Fire Commissioner Lloyd Ayers retired today after a 40 year career serving the Philadelphia Fire Department. Ayers graduated with Class 143 of the Philadelphia Fire Academy in 1974. He was first assigned in North Philadelphia and served in all corners of the city as he rose through the ranks. Ayers took over as commissioner in 2004. He now plans to travel with he wife. As always he keeps the memories of those firefighters lost and those currently on the job in his prayers. Deputy fire commissioner Derrick Sawyer will now take over the top spot.
